State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department's Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to [email protected].

Information about health care providers is on the agency's website. Click on "Health Care Provider Lookup" under the "Find it Fast" section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clark County

In June 2024 the secretary of health indefinitely suspended the certified nursing assistant license of Amy Lynn Kreuz (NC60519891) for three years. Prior to petitioning for reinstatement, Kreuz must complete eight hours of continuing education in the areas of theft awareness and law and ethics and pay a $250 fine. Kreuz pled guilty to third-degree theft and first-degree attempted criminal impersonation, both gross misdemeanors. In September 2021, Kreuz used a home care client's debit card to make multiple withdrawals.

In July 2024 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the licensed practical nurse license of Cory Churner-Tisdale (LP61005435). Churner-Tisdale failed to comply with a 2021 agreement by not complying with a monitoring contract.

Cowlitz County

In July 2024 the Board of Nursing (formerly the Nursing Commission) suspended the registered nurse license of Donna Marie Barkdoll (RN60556352). Barkdoll may not petition for reinstatement of her credential for at least 12 months. Prior to petitioning for reinstatement, Barkdoll must provide documentation that she has abstained from the use of any intoxicating, mind altering, or psychoactive substances for at least 12 consecutive months. Barkdoll failed to comply with a 2022 stipulation by being discharged from a monitoring program in February 2024 for failure to comply with her monitoring contract.

Douglas County

In July 2024 the secretary of health suspended the registered nursing assistant license of Jessica G. Soria (NA61428238). Soria may not petition for reinstatement of her credential for at least two years. Prior to petitioning for reinstatement, Soria must complete six hours of continuing education. In June 2023, Soria's employment was suspended because she borrowed money from a patient and had not paid the patient back.

Grays Harbor County

In July 2024 the secretary of health indefinitely suspended the dental assistant license of TreAnn Louise Andrew (D161384589). Andrew failed to comply with the 2023 agreement by failing to participate and complete the three-year WRAMP contract.

Kitsap County

In July 2024 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the licensed practical nurse license of Ethal Latoya McMillin (LP61292935). In June 2023, the Illinois Board of Nursing indefinitely suspended McMillin's licensed practical nurse license for dishonorable, unethical, or unprofessional conduct, failing to report any adverse final action, and for a criminal conviction.

Pend Orielle County

In July 2024 the secretary of health denied the application to reactivate the certified nursing assistant license of Alicia Ann Hobday (NC60642935). In November 2019, Hobday was convicted of first-degree rendering criminal assistance, a class B felony. In March 2021, Hobday reapplied for a credential to practice as a certified nursing assistant and failed to disclose her November 2019 conviction. Hobday failed to respond to a statement of charges by April 2024.

Pierce County

In July 2024 the Board of Nursing (formerly the Nursing Commission) suspended the licensed practical nurse license of Donald Russell Gifford (LP00043591). Gifford may not petition for reinstatement of his credential for at least 10 years. Prior to petitioning for reinstatement, Gifford must undergo a complete psycho-sexual evaluation. In March 2024, Gifford was convicted of indecent liberties, a class B felony, and third-degree assault, a class C felony, that involved patients at a facility where Gifford was employed as a nurse.

In July 2024 the secretary of health suspended the certified nursing assistant license of Natasha A'asa (NC60190740). A'asa may not petition for reinstatement of the credential for at least three years. Prior to petitioning for reinstatement, A'asa must successfully complete an ethics examination and pay a $500 fine. In August 2023, A'asa engaged in sexual contact with a patient. A'asa failed to submit a response to a Letter of Cooperation by November 2023.

Out of State

Oregon: In July 2024 the secretary of health indefinitely suspended the certified nursing assistant license of Tara L. Milburn (NC10065711). Prior to petitioning for reinstatement, Milburn must undergo a mental health evaluation and demonstrate that they meet relevant regulatory and licensure requirements for a certified nursing assistant credential. Between May 2021 and September 2022, Milburn was convicted of multiple gross misdemeanors, two class C felonies, and a misdemeanor. Milburn failed to respond to a statement of charges by January 2024.

Texas: In July 2024 the secretary of health indefinitely suspended the physical therapist compact privilege license of Bienvenido Tenedero Levardo (PT61328030-CP). Prior to petitioning for reinstatement, Levardo must attend and pass an ethics course program and pay a $1,000 fine. Levardo failed to respond to allegations of sexual misconduct by the due date.
