The City of Brockton continues to work hard to offer enhancements to our benefit plans while keeping the plans affordable for our retirees. This is why we are pleased to offer a new Voluntary Dental Plan to our Retirees which would take effect January 1, 2025.
The Dental Blue Freedom Plan features:
• Calendar year benefit maximum of $2,000 per member
• Two routine oral exams per calendar year
• Full coverage for x-rays per a set schedule
• 80% coverage for periodontal coverage and maintenance up to the calendar year maximum
• 50% coverage for complete or partial dentures and implants up to the calendar year maximum
• Accumulated maximum rollover benefits
This plan is 100% voluntary and is being offered to all retirees of the City and Schools and their spouses and children if applicable.
If you choose to enroll in this plan, you must complete the enclosed enrollment form and submit it to the City of Brockton Benefits Office on or before November 22, 2024.
You will be responsible for 100% of the cost of this plan. The rates are as follows:
• Individual: $51.12/month
• Family (2 or more members): $148.57/month
Deductions begin in your December pension payment, with enrollment changes going into effect January 1, 2025.
Once the Open Enrollment period ends, you must wait until next year's Open Enrollment or experience a qualifying event to make changes.
