The Cattaraugus County Department of the Aging/NY Connects is currently developing a 4-year plan and is seeking information regarding the needs, concerns and problems facing the citizens of our county. WE NEED YOUR HELP! Please complete the following survey and return as soon as possible. You may have also received an automated call, if you did the survey on the call you do NOT need to fill out the survey here. The call went out with part one August 13th and part two will go out via automated call on August 19th.
