Earn $250 by Scheduling Your On-Site Biometric Screening

Quest Diagnostics will be at the Student Alumni Center Gymnasium on Tuesday, September 4 from 8:00 am to 2:00 pm to complete the Biometric Screening for employees and spouses.

Employees can register/schedule for a screening by accessing my.questforhealth.com or new employees can register using the registration key: SOT.

On the Quest website you may either: download a physician screening form, or sign up for an onsite screening.

IMPORTANT!

Employees/spouses MUST complete the online health assessment to be eligible for any cash incentives -stateoftn.sharecare.com/ .

Once an employee and/or spouse completes Active Health's online health assessment, they can each earn up to $250 a year by completing certain wellness activities (if eligible). Each participant will be able to earn the maximum $250 per person ($500 annual maximum).
