Confirmation and pathogenicity of small copy number variations incidentally detected via a targeted next-generation sequencing-based preimplantation genetic testing for aneuploidy platform

Certain PGT-A platforms may enable the detection of inherited, small copy number variants (CNVs) with extremely high specificity without prior knowledge of parental status. Most CNVs in this data set were confirmed to be benign/likely benign or a variant of uncertain significance. Pathogenic/likely pathogenic CNVs associated with a broad range of phenotypic features may also be detected, although a reliable negative predictive value for small CNVs with current PGT-A technologies is unknown because of the many technical challenges.

Patterns of utilization of advanced practice providers in reproductive endocrinology: a 2023 national survey

This study evaluates the current utilization of advanced practice providers (APPs) within the field of reproductive endocrinology and infertility. A total of 201 APPs were surveyed through the American Society for Reproductive Medicine APP Professional Group. Advanced practice providers (APPs) are highly trained members of the care team but continue to be underused within reproductive endocrinology and infertility. Improvements in educational resources and/or the use of a formalized program to train APPs to their full scope of practice may help increase clinic efficiency and improve patient access to care.

The accompanying editorial Unlocking Potential: Advanced Practice Providers in REI discusses the importance of assessing the needs of the practice and referring to state regulations to gain a better understanding of all of the potential functions the APP can fulfill in an REI practice and how this can lead to benefit to the practice and the APP.

Refining endometrial assembloids: a novel approach to 3-dimensional culture of the endometrium

The goal of this study is to determine the most suitable ratio of organoids to stromal cells within assembloids and assess their viability over time, investigating to optimize their use in further studies. The 1:1 and 1:2 ratios of stromal cells relative to glands were found to be best suited to maintaining structural integrity, viability, and the apoptosis balance of cocultures from days 0 to 4. However, the significant decrease in viability and disappearance of stromal cells by day 10 in conventional culture point to the need for a more sophisticated model. Development of the hanging-drop assembloid model appears to be a promising solution because it addresses these issues and allows for prolonged culture with better-maintained cell structure and viability. Future experiments will focus on further refining the hanging-drop model to enhance its reliability and robustness. This includes evaluating the functional aspects of endometrial assembloids, such as hormone responsiveness and gene expression profiles. These improvements aim to establish a more physiologically relevant and long-lasting in vitro model for studying implantation dynamics and other aspects of endometrial biology.

Fertility, family building, and contraception in adolescents and young adults with sickle cell disease: a scoping review

Many adolescents and young adults are not counseled and are not knowledgeable about their fertility risks, family-building options, and contraception choices. Furthermore, previous counseling guidelines on these subjects are limited, leading to incomplete and highly variable discussions between patients and clinicians on these subjects. This review highlights counseling recommendations and areas in which more concrete guidelines and evidence are important for the standardization of comprehensive reproductive healthcare in the adolescent and young adult sickle cell disease population.
