State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department's Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to [email protected].

Information about health care providers is on the agency's website. Click on "Health Care Provider Lookup" under the "Find it Fast" section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Grays Harbor County

In June 2024 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the registered nurse license of Shonna M. Carnes (RN60092565). Prior to petitioning for reinstatement, Carnes must comply with a request to cooperate with the investigation, complete a Washington nurse practice act course, and complete continuing education in the areas of ethics of nursing practice, professional accountability and legal liability for nurses, patient advocacy and ethical nursing practice, patient assessment, and medication administration. In October 2020, Carnes was found sleeping during her shift. Carnes did not pass medications as scheduled and did not complete assessments or charting. Carnes failed to cooperate with the investigation by not furnishing a written response to the allegations.

Island County

In June 2024 the secretary of health indefinitely suspended the massage therapist license of Jason Alain Bohbot (MA00015302). Prior to reinstatement, Bohbot must complete an ethics examination in the areas of unprofessional conduct, professional standards, and boundaries. In February 2023, Bohbot admitted to having a sexual relationship with a client and was terminated at the spa he was working at. In April 2024, Bohbot failed to respond to a statement of charges sent by the board.

Jefferson County

In June 2024 the secretary of health and Alexander Lockwood (CG61213278) agreed to the suspension of his agency affiliated counselor license. Lockwood may not petition for reinstatement for at least 12 months. Prior to reinstatement, Lockwood must pay a $500 fine and complete 12 hours of continuing education in the areas of boundaries and law and ethics. In July 2022, Lockwood provided a patient, who is a friend, with money to purchase fentanyl, a schedule II drug.

King County

In June 2024 the secretary of health indefinitely suspended the massage therapist license of Leilei Wang (MA60970494). Prior to reinstatement, Wang must submit a plan for record keeping, patient or client charting standards and values, record storage and proper posting of licensure. Wang must also allow an investigator to do unannounced audits of at least 10 patient or client records. Wang failed to comply with a 2021 probationary stipulation in which she agreed to unannounced record keeping audits. In November 2022, Wang could not provide patient records during one of these audits and had expired licenses displayed throughout the business. In February 2024, Wang failed to respond to a statement of charges sent by the program.

Pierce County

In June 2024 the secretary of health indefinitely suspended the medical assistant-phlebotomist and registered medical assistant licenses of Jessica Ann Laughman (PC60351521, MR60710315). Prior to reinstatement, Laughman must contact a monitoring program and, if required, enroll in the monitoring program. In June 2023, Laughman pled guilty to fourth-degree assault-domestic violence after beating her child while intoxicated.

Walla Walla County

In June 2024 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the registered nurse license of Korina E. Casillas (RN60023215). Prior to reinstatement, Casillas must contact a monitoring program and, if required, enroll in the monitoring program, and Casillas must complete 10 hours of continuing education in the area of documentation for nurses. In January 2023, Casillas pulled narcotics for several patients but failed to document the administration or wastage of the narcotics. In July 2023, Casillas failed to respond to a request for a statement in response to the allegations.

Out of State

Arkansas: In June 2024 the secretary of health indefinitely suspended the home care aide license of Ce-Nedra Marie Teacher (HM61162007). Prior to reinstatement, Teacher must contact a monitoring program to determine if they will be required to participate and enroll in the monitoring program. In June 2023, Teacher admitted to her employer that she had taken oxycodone that was not prescribed to her. In January 2024, Teacher failed to respond to a statement of charges sent by the program.

Idaho: In June 2024 the Board of Physical Therapy indefinitely suspended the physical therapist license of Jerald Richard Olsen (PT60194827). In June 2019, Olsen was convicted of sexual battery and battery, both misdemeanors in Idaho. In February 2020, Olsen requested reinstatement of his credential to practice as a physical therapist in Washington state. In February 2024, Olsen failed to respond to a statement of charges sent by the board.
