State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department's Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Information about health care providers is on the agency's website. Click on "Health Care Provider Lookup" under the "Find it Fast" section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clark County

In July 2024 the secretary of health indefinitely suspended the massage therapist license of Misty Dawn Leonard (MA60071897). Prior to petitioning for reinstatement, Leonard must complete 12 hours of continuing education and pay a $1,000 fine. Leonard failed to respond to a DOH request for a statement addressing allegations that she practiced as a massage therapist with an expired license from June 2021 to March 2022, failed to provide medical records to two patients in April through August 2022, and injured and cursed at a patient in October 2022. Leonard also failed to provide medical records requested by DOH and failed to respond to an amended statement of charges by June 2024.

In August 2024 the secretary of health indefinitely suspended the agency affiliated counselor license of Alicia Kaye Stowe (CG61090405). However, if Stowe is convicted of first-degree sexual misconduct with a minor, a class C felony, then her credential to practice as an agency affiliated counselor is suspended for 10 years. Prior to petitioning for reinstatement, Stowe must complete a psycho-sexual evaluation and pay a $5,000 fine. In January 2023, Stowe was charged with first-degree sexual misconduct with a minor for having a sexual relationship with a minor patient in March 2021.

King County

In August 2024 the secretary of health indefinitely suspended the massage therapist license of Yue Wu (MA60750271). Prior to petitioning for reinstatement, Wu must complete six hours of continuing education. In March 2023, Wu's patient files lacked information regarding age of patient, health history, techniques and modalities applied, location of the body that received massage, and written informed consent to treat.

Pierce County

In August 2024 the Dental Quality Assurance Commission indefinitely suspended the dental assistant license of Anita Eternitee Wayne (D161309286). Prior to petitioning for reinstatement, Wayne must pay a $500 fine and complete six hours of continuing education. In November 2023, Wayne was placed on the Department of Social and Health Services long-term care abuse and neglect registry for physically abusing a vulnerable adult. Wayne is prohibited from being employed in the care of or having unsupervised access to vulnerable adults. Wayne also failed to respond to a statement of charges by June 2024.

Snohomish County

In July 2024 the secretary of health permanently revoked the independent clinical social worker and substance use disorder professional licenses of Brian Keith Hall (CP00005232, LW60492025). Hall may not seek reinstatement of those credentials. From October 2017 to August 2018, Hall sexually abused three minor clients. In August 2018, Hall was arrested on suspicion of indecent liberties, a class B felony, related to his counseling of a ten-year-old child. In September 2022, Hall was convicted of fourth-degree assault, a gross misdemeanor.

Spokane County

In August 2024 the secretary of health indefinitely suspended the mental health counselor license of Lynn A. Fraley (LH00007678). Prior to petitioning for reinstatement, Fraley must pay a $3,000 fine. In July 2021, the state of Idaho revoked Fraley's clinical professional counselor license because of improper billing practices. Fraley failed to respond to a statement of charges from DOH by November 2023.

Thurston County

In August 2024 the secretary of health suspended the massage therapist license of Yong Huang (MA60655888) for two years. Prior to petitioning for reinstatement, Huang must attend and pass an ethics course. In July 2023, Huang was found to be out of compliance with a 2023 stipulation to informal disposition by failing to keep proper patient records. Huang also failed to respond to a statement of charges by April 2024.
