State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department's Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to [email protected].

Information about health care providers is on the agency's website. Click on "Health Care Provider Lookup" under the "Find it Fast" section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In September 2024 the secretary of health summarily suspended the certified nursing assistant license of Aylin Guadalupe AispuroMoreno (NC61263192) pending further legal action. In May 2023, Moreno was placed on the Department of Social and Health Services long-term care abuse and neglect registry after financially exploiting a client. Moreno is prohibited from being employed in the care of or having unsupervised access to vulnerable adults.

Clark County

In December 2023, the secretary of health indefinitely suspended the chiropractor license of Mark S. Larue (CH00003525) for at least 10 years. Prior to petitioning for reinstatement, Larue must pay a $5,000 fine, undergo a psycho-sexual evaluation, complete an ethics and boundaries assessment, and pass a jurisprudence exam. Larue was convicted of one count of second-degree rape, a class A felony. Larue was also convicted of one count of indecent liberties without forcible compulsion, a class B felony. Larue failed to appear at a scheduled pre-hearing conference on June 2023.

King County

In September 2024 the Pharmacy Quality Assurance Commission indefinitely suspended the pharmacist technician license of David Bernard Dilworth Jr. (VA60681478). Prior to petitioning for reinstatement, Dilworth must enroll in a substance abuse evaluation program. In December 2023, Dilworth was seen taking two bottles of Promethazine-Codeine Syrup from the pharmacy he was working at. From January to December 2022, approximately 46 bottles of Promethazine-Codeine Syrup were unaccounted for during times Dilworth was working as a technician at the pharmacies. Dilworth failed to respond to a statement of charges by June 2024.

In September 2024 the secretary of health indefinitely suspended the marriage and family therapist license of Caitlyn B. King (LF60923999). Prior to petitioning for reinstatement, King must comply with any and all Department of Health investigations involving her marriage and family therapist license. King must also comply with any and all conditions to renewing her license to practice as a marriage and family therapist. Between November 2021 and May 2022, King allegedly provided therapy services with an expired license. King failed to appear at a scheduled pre-hearing conference in June 2024.

Okanogan County

In August 2024 the secretary of health indefinitely suspended the chiropractor license of John Herbert Hammer (CHIR00003140). Hammer failed to comply with his 2022 agreed order by failing to pass the jurisprudence exam and failing the audit.

Skagit County

In August 2024 the secretary of health indefinitely suspended the home care aide license of Derek Blake Kucala (HM61152840). Kucala failed to comply with the terms and conditions of his 2022 agreement by failing to provide a performance evaluation or unemployment declaration by June 2024.

Spokane County

In September 2024 the secretary of health summarily suspended the registered nursing assistant license of Anthony MauriceStevens (NA60792253) pending further legal action. In March 2024, Stevens was placed on the Department of Social and Health Services long-term care abuse and neglect registry for emotionally abusing and improperly restraining a vulnerable adult. Stevens is prohibited from being employed in the care of or having unsupervised access to vulnerable adults.

In August 2024 the secretary of health and massage therapist Matthew David Herrera (MA60414852) entered an agreed order. Herrera surrendered his credential to practice as a massage therapist. In October 2023, Herrera engaged in unprofessional misconduct with a patient for no clinical purpose and without the patient's permission.

Whitman County

In September 2024 the Pharmacy Quality Assurance Commission suspended the pharmacy assistant license of Erin M. Donceel (VB61471624) for at least five years. Prior to petitioning for reinstatement, Donceel must pay a $500 fine and enroll in a substance abuse evaluation program. In November 2023, Donceel stole prescriptions for Oxycontin and Oxycodone from the clinic where she worked. Donceel failed to respond to a statement of charges by July 2024.

Out of State

Hawaii: In August 2024 the Board of Naturopathy and naturopathic physician Allison Anne Hofmann (NT60209747) entered an agreed order. Hofmann surrendered her credential to practice as a naturopathic physician. Hofmann diagnosed patients with unreliable methods without substantiating those diagnoses using other credible methods, tools or referrals to appropriate specialists.
