State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department's Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Information about health care providers is on the agency's website. Click on "Health Care Provider Lookup" under the "Find it Fast" section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clark County

In July 2024 the secretary of health summarily suspended the certified nursing assistant and medical assistant-phlebotomist licenses of Nadya V. Andryushin (PC60419291, NC10073635) pending further legal action. In October 2023, Andryushin was placed on the Department of Social and Health Services' long-term care abuse and neglect registry. As a result, Andryushin is prohibited from being employed in the care of or having unsupervised access to vulnerable adults.

Douglas County

In June 2024 the secretary of health indefinitely suspended the x-ray technician license of Basilisa Jimenez Hernandez (XT61345352). Prior to reinstatement, Jimenez Hernandez must pay a $250 fine. In April 2021, Jimenez Hernandez practiced as a radiologic technologist without an active license.

King County

In June 2024 the secretary of health suspended the acupuncturist license of Gon Kim (AC00002784). Kim may not petition for reinstatement for at least two years. Prior to reinstatement, Kim must undergo a complete psycho-sexual evaluation and pay a $2,000 fine. Between August 2022 to December 2022, Kim engaged in sexual abuse of a patient on at least two occasions. Additionally, Kim failed to obtain consent to treat sensitive areas and failed to explain a health care-related necessity related to that treatment.

Kitsap County

In April 2024 the secretary of health summarily suspended the certified nursing assistant license of Angie M. Kelley (NC10063118). In February 2024, Kelley was placed on the Department of Social and Health Services' Adult Protective Services long-term care, abuse and neglect registry. As a result, Kelley is prohibited from being employed in the care of or having unsupervised access to vulnerable adults.

Mason County

In June 2024 the secretary of health indefinitely suspended the agency affiliated counselor license of Vincent Michael Dalseg (CG61315710). Prior to reinstatement, Dalseg must contact a recovery and monitoring program and, if required, enroll in the monitoring program. In November 2022, Dalseg self-reported that he had relapsed and resumed using alcohol and controlled substances.

Pierce County

In June 2024 the Board of Nursing (formerly the Nursing Commission) summarily suspended the licensed practical nurse license of Brandon Laurence Rucker (LP61326755) pending further legal action. In July 2023, the Nevada State Board of Nursing revoked Rucker's licensed practical nurse license. Rucker failed to meet the health needs of a patient by failing to observe the symptoms of a patient and recording that information or reporting changes, and endangered the safety of the public, patients, clients or co-workers by making actual or implied threats of violence.

In July 2024 the Chiropractic Quality Assurance Commission indefinitely suspended the chiropractor license of Bo Robert Mullen (CH60582933). Prior to reinstatement, Mullen must pay a $5,000 fine and provide a written COVID-19 proclamation compliance plan. In September 2021, Mullen, a co-owner of Harbor Life Chiropractic, did not comply with multiple Washington legal codes requiring the use of masks during the COVID-19 pandemic.

Thurston County

In June 2024 the Board of Nursing (formerly the Nursing Commission) summarily suspended the registered nurse and advanced registered nurse practitioner licenses of Percy Lee Lynchard (RN60049829, AP60800360-NP) pending further legal action and amended the statement of charges of unprofessional conduct against Lynchard. Between 2018 and 2024, Lynchard allegedly practiced below the standard of care in treating multiple patients. Lynchard allegedly failed to address the requirements of the pain management rules when prescribing controlled substances, failed to refer patients to specialists when indicated, failed to document that proper diagnostic criteria was considered prior to diagnosing patients with ADHD, inappropriately dosed ADHD medication, failed to use proper risk screening tools in the initial assessment of patients or monitor patients for conformity with treatment regimens, and engaged in off-label prescribing of substances without clear documentation that patients were informed of the lack of FDA approval.
