Help shape a plan to improve community health and wellness in Berkeley

Help inform a plan to create a healthier, more equitable community by filling out an online survey by October 11.

The survey from our Department of Health, Housing, & Community Services will ask about your perspective on the current state of individual and community health in Berkeley, which health issues are most important to you, and ideas for ways to address these issues.

We're looking to get input from a wide range of those who live and work in the Berkeley community for a complete understanding of the state of health across our city. While many people have health insurance and access to health care, we are looking for possible ways to improve or increase community health programs and services the City provides or facilitates.

We'll take that input to help set priorities for projects to improve health outcomes, especially for those from historically underserved areas and identities.

Please complete the survey by October 11 and share it with others.

The survey will take about 15-20 minutes to complete and is available in English, Spanish, Korean, and Chinese. If you complete the survey, you can be entered to win one of ten $50 gift cards.

The survey will ask you about which health and wellbeing issues matter most to you and why, including:

  • Community safety
  • Environmental health
  • Jobs and money

You can also get a paper copy of the survey to fill out in person at a library branch, senior center, or City Hall:

Please share widely among any who live or work in Berkeley. The more responses we get, the broader the set of experiences we'll capture.

Help improve health equity in our community

Your survey responses will help set priorities for health and wellness services. Where the survey shows greatest need, for example in youth mental health support or transportation for seniors, you may see new or improved services.

This effort is part of the Berkeley Wellness Blueprint Project, a multi-year effort to create a community-shaped vision and action steps to improve health and equity in Berkeley.

Recommendations from the Berkeley Wellness Blueprint Project will help the Health, Housing, and Community Services Department, and its partner organizations, better align services with community needs.

The Department offers a range of services to support a thriving Berkeley, including free vaccinations,mental health services, and senior services.
